We’re here to help all patients understand their benefits, schedule appointments, and fulfill their eye doctor’s orders. Our opticians can give you guidance in picking out your perfect pair of eyewear, and our friendly office staff is knowledgeable about billing and benefits coverage.

Meet The Doctors

Daniel R. Hock, OD, FAAO, FCOVD

Dr. Hock provides routine vision exams, medical eye care and specializes in developmental optometry in which he diagnoses and treats vision problems that affect learning.

He received his undergraduate degree from Colorado State University and his doctorate degree in 1995 from Pacific University College of Optometry in Oregon.

Dr. Hock serves as a committee member of the Children’s Vision Task Force, participates as an InfantSee Provider, and is in the Sight for Students Program.

He treats medical conditions that affect vision such as glaucoma, diabetes, and macular degeneration. He provides consultations for primary care physicians and specialists. Dr. Hock has managed refractive surgery patients since 1995.

Dr. Hock feels very fortunate to practice in the mountain community in which he was born and raised. He enjoys the Colorado outdoors and spending time with his wife and children.

Nicole Wellensiek, OD

Dr. Wellensiek will provide your evaluative, pre-operative, and post-operative care at Evergreen Vision Clinic.

She received her optometry degree from Indiana University School of Optometry. After graduating in 1997, she practiced in her home state of Nebraska for seven years, specializing in contact lens fitting, primary care, and refractive surgery co-management

Dr. Wellensiek moved to Colorado in 2004 and has worked in a variety of clinical settings including two years at a refractive surgery center. Here she had the opportunity to work with a wide variety of patients as they experienced the live-changing event of refractive surgery.

She lives in Evergreen with her husband and two children. Dr. Wellensiek enjoys running, hiking, and spending time with her family.
